根据卫生福利部104年国人死因统计，十多年来，肺癌一直是癌症死亡榜首。台北荣民总医院胸腔部主治医师蔡俊明表示，肺癌患者很容易出现脑转移，由于大脑有血脑屏障，因此一般药物很难进入大脑，大脑常成为肺癌细胞的「避难所」。 找出关键基因　选择适合药物 肺癌依病理特征主要分为小细胞肺癌与非小细胞肺癌，约九成患者为非小细胞肺癌。肺癌治疗目标是要抑制肿瘤生长，目前已知有许多关键基因可作为治疗标的，除较为人熟知的EGFR（表皮生长因子接受器），2007年新发现的ALK（间变性淋巴瘤激酶）也有对应标靶治疗药物，医师建议，患者应先进行基因检测，找出关键致病基因，再搭配合适的药物，以增进治疗效果、减少副作用发生。 以EGFR阴性的肺癌病友来说，约10-15%的患者有ALK基因易位突变现象，研究也发现，ALK基因易位、突变的肺癌患者，以没有抽菸或只有轻度抽菸史的族群与较年轻、平均年龄52岁族群、女性族群为主。 新一代ALK肺癌标靶药物　有助增加存活期 蔡俊明医师表示，目前针对ALK肺癌已研发出新一代标靶用药，可专一抑制ALK活性，直接锁定、消灭癌细胞，在今年(2016年)6月ASCO发表的一项日本大型临床试验显示，新一代标靶药物的反率高达94%，中位数无恶化存活期已超过29个月，且持续延长中。若做为第二线标靶治疗，反应率仍高达52%，中位数无恶化存活期亦达8.2个月。 蔡俊明医师进一步说明，多项国内外临床试验显示，新一代ALK标靶治疗安全性很高，服用后腹泻、反胃、便秘、呕吐、疲累等副作用发生比率较低，不过长时间用药，也可能产生抗药性。 新型药物可进入大脑　歼灭癌细胞 另外，目前市面上的抗癌药物不易进入脑部，尽管对肺部肿瘤有效，但治疗后仍有四成患者出现脑转移，医师指出，新一代ALK标靶药物也可穿过血脑障蔽，进入大脑组织，有效对付癌细胞，甚至可预防、延缓脑转移。 定期接受健康检查　才能及早发现病灶 蔡俊明医师呼吁，在日常生活中应多注意肺癌危险因子；最好定期进行健康检查，及早发现、积极治疗。而肺癌患者也应正向积极，接受EGFR、ALK等基因筛检，并配合医师选择适合治疗策略。 订阅【健康爱乐活】影音频道，阅读健康知识更轻松 加入【】，天天关注您健康！LINE＠ ID：@ ： /cancer/article/30705/肺癌容易脑转移　基因检测标靶成灭癌利器 关键字：肺癌, ALK, 标靶药物, 蔡俊明, 台北荣总, 血脑屏障
