no 是形容词，形容词接名词所以talk动词要变成名词形式，也就是talking，而not是副词可以直接修饰动词，所以talk不要变形。

劳拉替尼说明书 alk突变三代靶向药劳拉替尼 药品名称：劳拉替尼（Lorlatinib） 商品名称：Lorbrena 适应病症：ALK阳性非小细胞肺癌 给药途径：口服 药物类型：靶向治疗药物 药物规格：25mg，100mg；片剂 中国是否上市：是 Lorlatinib（劳拉替尼）是辉瑞公司开发的一种新型、可逆、强效小分子ALK和ROS1抑制剂，其对ALK已知的耐药突变均具有很强的抑制作用，因而被誉为第3代ALK抑制剂。 2018年11月2日，FDA批准了Lorlatinib劳拉替尼，用于ALK阳性转移性非小细胞肺癌（NSCLC）患者，其疾病在克唑替尼和至少一种其他ALK抑制剂（阿来替尼、色瑞替尼）治疗后进展的患者。 无论既往用过几种靶药或者化疗过，劳拉替尼始终能够作为ALK耐药最终保底的药物。 劳拉替尼也在各线治疗展示强劲的炉内控制力。的NCCN指南将劳拉替尼列为三线保底治疗推荐。 劳拉替尼在Ⅱ期临床试验中显示出对肺肿瘤和脑转移瘤的持久的疗效。先前接受过2-3种ALK抑制剂治疗后的患者客观缓解率达39%，颅内缓解率达48%。 Lorlatinib的获批基于Ⅱ期临床试验，我整理完整数据如下： 对于ALK阳性的初治NSCLC患者，ORR高达90%，颅内ORR达到了75%；对于ALK阳性且先前接受过克唑替尼治疗的患者，ORR为69%，颅内ORR达到了68%；对于ALK阳性且先前接受过克唑替尼以外的其它ALK抑制剂治疗的患者，ORR为33%，颅内ORR达到了42%；对于ALK阳性且先前接受过2-3种ALK抑制剂治疗的患者，ORR为39%，颅内ORR达到了48%；对于ROS1阳性的经治患者，ORR为36%，颅内ORR达到了56%。 用药前，先基因检测 对于肺癌靶向药的选择，患者必须清楚的是用药前一定要先检测，精准靶向治疗才是晚期NSCLC诊疗的大势所趋。我们要检测，不要猜测。没有ALK基因突变，那么请放弃使用阿来替尼。 ALK融合基因检测的材料主要来源于手术标本、CT引导下活检组织、纤维支气管镜活检组织、胸水细胞包埋组织等。ALK基因重排常用的检测方法目前常用的有：荧光原位杂交、免疫组织化学、反转录-聚合酶链式反应。 上述方法于目前大部分医院病理科均可开展，各大基因检测公司也能检测。但是，患者选择基因检测机构时一定要注意，选择可信赖的基因检测公司。因为不同的基因检测公司，检测的准确度是不同的，要选择肿瘤医院或者国家机构认证的大型基因检测公司。了解国内外基因检测机构登录全球肿瘤医生网咨询。 劳拉替尼耐药后，怎么办？ 研究表明，几乎所有的靶向药治疗后都会出现耐药问题，只是有的人耐药早，有的耐药晚。 那么，患者耐药后，一定要再重新进行一项基因检测，寻找发生耐药的机制，对症治疗。 若发生的仍为ALK其他位点的基因突变，可以换用其他ALK抑制剂。由于劳拉替尼是其他ALK抑制剂的保底药物，再没有其他标准选择的时候可以寻找临床试验，尝试新的治疗机会。 若发生的是驱动基因旁路激活突变，包括EGFR旁路激活，KIT基因扩增，或其他驱动基因突变；若是EGFR、KIT或其他驱动突变导致的旁路激活，需要针对这些靶点突变用相应的靶向药。 耐药原因不明的，也可以再取组织进行活检，看看是否是发生了组织学变化，这时候可能就需要化疗或者医生推荐的其他治疗手段。 用法用量 建议剂量：口服每天一次，每次100mg，每天在同一时间服用。忘记服用时可补服，除非距离下一次服用的时间少于4小时，不可同时服两次分量。 剂量减少调整：第一次剂量减少可调整至每天一次75mg；第二次剂量减少可调整至每天一次50mg。若无法承受每天50mg的剂量，则永久停止。 呕吐的情况下不用补服，照常在下次才服用就可以。 用药注意 可以与食物同服，也可不同服。 药片整个吞下，不要压碎或咀嚼。 许多药物都会影响阿来替尼疗效，包括处方药和非处方药、维生素和中药产品，服用其他药物前请与主治医生沟通。 服药期间需有效避孕，孕期及哺乳期禁止服用劳拉替尼。 若有过敏反应的迹象，如荨麻疹、呼吸困难、脸部，嘴唇，舌头或喉咙肿胀，尽快就医。 常见不良反应 水肿，周围神经病变，认知功能，呼吸困难，疲劳，体重增加，关节痛，情绪影响和腹泻。
