头痛怎么办

  老实说,我最怕的就是问诊的“头痛”和“女性经带”,这两个问题是中医里问的最常见的,也是最复杂,辩证最多的。群里也经常有人问头痛啦,怎么治疗,一听我更是头痛!咱们互相揉揉吧。你说一个头痛问我怎么治疗?我告诉你中医里头痛有20多种辩证,读完后大家就知道了,无米之炊,真是难为樵夫啊。(谦虚点,低调点。咱不敢说巧妇!),要求:读完这篇文章大家可以正确诊断每种头痛,并给出针灸治疗穴位。下期我会再把女性经带胎产辩完,下面开始吧.  一.根据头痛性质划分:  首先,一来就上重点,阴阳为中医之本,那么头痛也分阴阳,各位记住:根据前面说过的阴阳对立的原则,白天的头痛,不管是慢性的还是急性的,或者白天加重的全部归于气病或者阳病,所有晚上的头痛,不论是否加重,慢性急性的全部归于血病或者阴病,记住了吗?气和阳的病在白天发作或者加重,血和阴的病在夜晚发作或者加重!!此为头痛之大原则!后面将谈到的所有头痛辩证都基于阴阳原则,若有冲突,以阴阳对立原则为准。如:一个女病人因为丈夫时常在外加班应酬而时常怀疑丈夫有外遇。症见失眠头晕,晚上盗汗严重,心悸乏力,晚上头痛加重,朋友们,马上想到阴病或者血病。  其次,按照虚实性质(严重还是不严重)来判断下,头痛如果是隐痛(anguish pain)和钝痛(dull  pain)的,缓慢性的,归于虚证,解释下钝痛,钝痛者,是相对于实痛而言的,不像刀割,针刺那种是定点的,它不太尖锐。钝痛属于一种范围内的痛,涉及范围比锐痛广泛,诊断隐痛和钝痛多为血虚气虚,肝血虚,肾虚等。再看如果头痛是锐痛,剧痛的(Sharp  pain),,并且是突然性发作的,属于实证,症多见肝阳上亢,肝火肝风,痰浊,胃火,血瘀,解释下锐痛和剧痛,记住这两者是突然性的深度头痛。  再来,头痛有沉重感的判断为痰湿,之前说过了,看到“重”马上想到痰湿,《内经》不是说“因于湿,首如裹”嘛。那么痰湿大家都知道是两个概念,这里很重要,一大半的中医只知道你头疼痛加沉重就是痰湿,但你问他到底是痰还是湿,他就支支吾吾了,让我教大家,痰者伴有眩晕眼花,湿者,只有头重和疼痛。  牵涉到后颈部僵硬的,如果突然性头痛伴有颈部僵硬的属于外感风寒(见我的六经之太阳经症);如果长期头痛伴有颈部僵硬的肝阳上亢(高血压病人常见,经常头痛,脖子紧)。  抽痛(throbbing  pain)(有牵涉性的),诊断为肝风,什么叫抽痛,感觉过失恋吗?那种一阵一阵心痛的感觉,把它转移到头上就是抽痛,(完了,暴露了!各位,我为了让大家了解中医术语而牺牲自我成全大家的精神是不是该来点掌声啊?)  刺痛(stabbing pain),定在一个点上的痛,像用针刺着一样的痛,诊断为血瘀。  胀痛(distending pain),临床多见,不但痛,还有紧,胀的感觉,诊断气滞,多见肝阳上亢和肝火上炎。  空痛(empty  pain)为肾虚,脑袋空空的,没什么思想,只觉得痛,用什么穴位?哈哈,脑袋空当然是用胆经的脑空(引肾经入脑),再配补肾的穴位,比如太渊啊,足三里啊,(看来今晚要扎一下了,写的太多太快,已经感觉自己脑袋空空了)  突然松懈后头痛加重(西医叫周末头痛症,weekend  headache),由于肝阳上亢造成的,星期一到星期五都没事,一到星期六开始头痛,星期天更痛,星期一早上准备上班时,头痛就好了。用我的家乡话说叫装样性头痛,一到五装样给老板看,所以很紧张,一下班真实面貌就露出来了。  突然兴奋后头痛加重属于同样的肝阳上亢  吃酸性食物后(柑橘类水果)头痛加重,也是肝阳上亢  头钝痛,房事行为后头痛好转----肝火上炎,各位,有没有谁的老公要求行房事是因为头痛的?说:“哎呀,今天头痛的厉害,咱们早点上床吧?”大家不要笑,这种情况的人很多,虽然没抱怨头痛,但看完你就知道为什么了,不是人色,是病啦!!  二 头痛和女性月经的关系  一般而言,女性头痛要比男性多,这是因为女性的生理原因,我连腹痛一起总结,临床上问女性经常配合着问。这样便于大家记忆背诵!!  经前头痛——肝阳上亢 (阴阳不和) 经前腹痛-----气滞血瘀(阴阳不和)  经期头痛或头痛加重----肝火,血瘀(实证) 经期腹痛------血热,血寒(实证)  经后头痛-----血虚 (虚证) 经后腹痛------血虚(虚证)  问:一女经后头痛,经后腹痛的,连起来看,是不是血虚?  一女经期头痛,经前腹痛的,是不是血瘀伴有肝气淤滞或者肝火上炎?  一女经前头痛,经期也头痛的,是不是肝阳上亢或者肝火?  一女经前腹痛,经期也腹痛的,是不是血瘀?要么寒,要么热,舌淡苔白的血寒,舌红黄苔的血热?  会玩了吗?  在临床时,我们按照头痛性质问病人,但有些病人自己都说不清到底是怎么痛的,反正就是痛,甚者还反咬你一口,你问多了他们还说“你不是医生吗,我告诉你我头痛你就治疗头痛,我怎么知道什么钝痛,胀痛的”,对于这种肚里有屎又不想拉的病人,难道我们就没办法了吗?不,这样的话咱们换种方式,不问痛的性质了,直接问痛的部位就搞定,下面进入头痛部位的辩证,没看我前面《总结六经辩证》的,请先看完,因为头痛按部位区分可以和我之前的六经辩证互相做补充。  1, 枕骨疼痛(occipital  pain),放射到颈部的-------太阳头痛,常见于外感风寒和肾虚.为什么肾会痛在后脑勺,学中医一定要把不懂的地方搞懂,你死记是玩不走的,所以说中医要悟性,太阳头痛是足太阳膀胱经,循行上后脑勺的,膀胱经和哪条经相表里?肾经!是不是互相沟通了。  重点来了,大家知道根据不同的头痛部位,我们中医是不是有不同的引经药?来一起治疗吧!  太阳头痛的引经药——羌活  太阳头痛的穴位(对比我的六经辩证之太阳证)——后溪配昆仑(太阳头痛的远端穴)。  我很少用局部(阿是)取穴,远端配穴是我的最爱,为什么?效果好呗。就像你垂直丢个石头在水里,声音很大,接着就石沉大海了,我们叫什么?外强中干,哈哈,但是你们打过水漂吗?让石子在水上跳五六下,打的好的话,可以有十多个水花,我们叫什么?循经传导,效果是不是更雷人(不知道雷字的用法用对没有)?  重点又来了,一个病人说他太阳穴这里痛,请问大家这是哪条经的病症?太阳经吗?NO,太阳穴头痛是偏头痛,偏头痛是属于少阳经头痛,记住啦  2, 前额痛,眉棱骨痛(forehead  pain)------阳明头痛,足阳明胃经,那大家知道了,是中焦脾胃的原因导致的。隐痛为胃虚血虚,剧痛为胃热,前额头重的痰湿  阳明头痛的引经药——白芷,葛根  阳明头痛的穴位——合谷,内庭  3, 巅顶头痛(parietal headache)-----厥阴头痛,常见于以下三种  1) 肝血虚:躺下有所缓解  2) 寒凝肝脉:伴有寒象  3) 肝阳上亢:剧烈疼痛  重点:如果是从枕骨处痛起来的,逐渐上移巅顶的,它的本是太阳经引起的,所以是太阳经头痛。那引经药事藁本,羌活。  厥阴头痛的引经药——吴茱萸(上达巅顶),所以吴茱萸汤主治什么?寒凝肝脉造成的巅顶痛。  厥阴头痛的穴位-----至阴,太冲  4,  偏头痛(migraine)-------少阳头痛,多是肝胆病,为什么肝叫少阳头痛?足少阳胆经,肝胆相照,互为表里,就像之前说的肾经会有后头痛。  重点:头痛伴有眼球后面痛的,隐痛肝血虚,剧痛肝阳上亢  少阳头痛引经药-------柴胡,黄芩(镇肝阳上亢的要药),川穹(头痛不离川芎)  少阳头痛的穴位——足临泣,外关,记得吗?群里有人问过的,我还告诉大家这是六经辩证的少阳证来鸟。  5, 全头痛(headache)------少阴头痛,整个头都痛,慢性发作的空痛是肾虚,急性发作的抽痛是外感风寒。  少阴头痛的引经药:细辛,独活  少阴头痛的穴位:无穴可配,风寒用三风穴,肾虚用补肾穴。  顺便把头晕也搞定了,说天也要说说地,说男也要说说女,说头痛也要说说晕。  眩晕:记住风火痰虚导致眩晕就OK拉,简单吧?  1, 感觉天旋地转的,每样物品都在动的——内风  2, 眩晕感觉头部沉重的——痰浊内阻  3, 疲倦后加重的——气虚血虚  4, 突然晕为实证 渐进晕为虚证。求采纳为满意回答。
