大家好，我是一名肾内科医生，在工作中接触最多的就是透析的病人了，我要说的是透析并不意味着生命开始倒计时，因为人不可能会长生不老，如果非要比较，人从呱呱落地的那一刻开始，每过一小时、一天、一年都是离死亡更进一步。 一、什么是血透？ 根据我推测题主的想法，题主可能觉得血透会是一件非常恐惧的事情，病人每天生活在痛苦中，其实不然，血透的病人只是因为治疗需要每周需要去医院的透析中心进行透析，其实平时生活上和大家相差的不会那么大的。 （1） 透析模式： 言归正传，“血透”这个词语其实是一个缩写，标准的说法是“血液透析”，当然了，这只是一个统称，它下面有很多种模式，例如：血液透析、血液滤过、血液透析滤过、单纯超滤；如果根据清除毒素能力来划分，又可以分为低通量透析、高通量透析；如果根据透析时间来说，又分为间断血液透析、持续性肾脏替代治疗。 （2） 透析原理 ：一次完整的透析过程必须要以下两样东西，即透析机和透析膜。大家可别小看透析膜，它能清除体内的毒素、水分、电解质等物质。根据透析膜的不同和透析模式的不同，它将发挥它扩散、对流、吸附等作用进行工作。 二、关于透析的人生思考？ 这一点上我觉得理论讲的太多也没意思，我就来讲讲我们透析中心的几个案例，具体透析的人生如何，那就留给大家去思考吧。 案例1、 这位透析病友年纪35岁，以前有肾炎病史，自己也不重视，尿毒症了也只能透析治疗了，但他并没有放弃自己的事业，生病以前他是一个非常肯拼搏的人，自己整了一个小作坊，买了几台机器，接订单给别人加工零件，没日没夜的做，透析后自己是不做事了，聘了几个工人，虽然收益可能没以前好了，但是人再也没有以前那样心事重重了，因为他明白了，赚再多的钱也没有 健康 的身体更宝贵，透析后听医生的话，由于年纪也不大，目前基本没什么并发症，已经在省级医院排队准备移植了，每次来透析都挎个老板包，心态很好，精神焕发。 案例2、 这也是一位男性透析病友，26岁诊断尿毒症，透析一年后就已经肾移植了，然而好景不长，在后面的一段时间由于肺部感染移植那个肾脏又没用了，然后又开始透析治疗，家庭条件不错，目前也在等肾源准备再次移植，由于移植对身体条件也是有要求的，例如心功能要达到多少，贫血怎么样，都是需要参考的指标，不用我说，每次都很主动，哪个哪个指标要复查了，体重控制的好不好，因为他知道，移植后的生活和透析的生活质量是不一样的。 案例3 、这是一位年轻的女性，在二十多岁的时候在医院就考虑是肾炎，死活不肯肾穿刺，过了一段时间，肾功能变得差起来了，跑到上海，还是要穿刺，穿刺后考虑是IgA肾病，人那时候已经肿的很厉害了，蛋白也低，方案就是用激素，效果是还不错，但年轻小姑娘爱美，觉得激素吃了副作用大，可能以后还影响生小孩，这个时候激素的疗程还没到，在肿退了以后就把激素全停了，听别人介绍，哪里有个老郎中治疗这一块的病特别厉害，药到病除，排队找他看病的不知道有多少，然后就开始在那个郎中那里吃草药调理，后面送到医院来了，是因为心衰了（尿毒症的并发症），肾脏彩超做出来肾脏已经萎缩了，后面开始做腹透，腹透做了几年现在在做血透，每次都是她的老父亲风里来雨里去的接送，真的是父爱如山。 案例4、案例6、案例6、案例7、8......... 三、总结： 每个人都是不一样的个体，所以每个人在透析后的人生感悟也不一样，生命并不会因为你尿毒症透析了开始倒计时，看的是每个人对待生活的态度，是踌躇、是期待、是遗憾，还是什么？不论是谁，都决定不了生命的长度，但可以决定生命的宽度，可以在有效的生命里做更多有意义的事情，俗话说的好，阎王要你三更走，谁敢留你到五更，不活在过去，也不活在将来，乐观的活在当下的每一天。 作者寄语：这是一个开放题，可能点进来看的有不少尿毒症的患者，每个人都有自己对待的态度，你的经历就是属于你的故事，欢迎大家在评论区说出自己的感受和想法，让肾友们不再感到孤单。 对于很多种疾病来说，发展到一定程度都会引起肾损伤，像我们常见的慢性肾脏疾病、糖尿病、高血压等，发展到终末阶段，患者可能就会出现明显的厌食、恶心、呕吐等，这时候我们要警惕肾脏是不是已经起不到正常的代谢作用，当我们化验时血尿素氮 28.6mmol/L，血肌酐 707ummol/L，血钾 6.5mmol/L，或者出现严重的水肿，代谢性酸中毒等时就需要血液透析，否则会引起严重的并发症，引起生命危险。总而言之，血液透析就是使我们肾脏原本能正常排出的而现在排不出的东西过滤出去，维持人正常的生命活动。但是开始透析并不意味着生命的倒计时，患者要积极配合医生治疗。 血液透析，简称血透，通俗的说法也称之为人工肾、洗肾，是血液净化技术的一种。利用半透膜原理，通过扩散、对流，将体内各种有害以及多余的代谢废物和过多的电解质移出体外，达到净化血液、纠正水电解质及酸碱平衡的目的。目前是我国最常采用的终末期肾病治疗手段。血液透析有诸多优点：透析效率高、短时间可以清除体内较多毒素；准确完成设定的脱水量，清除体内多余水分；治疗由医护人员操作，患者相对比较省事；技术开展时间长，多数县级以上医疗单位均可开展。 血液透析后生命能持几年? 尿毒症病人的存活年限，理论上讲，只要是规律透析，并不断调整药物，积极治疗并发症，并结合营养饮食等，患者的寿命并无明显缩短。据报道，亚洲地区从有透析开始，就有病人不断刷新存活年限，现在已有存活40年以上的病人了(透析技术普及也就40多年)。各地血液透析科根据成立的早晚也都有和科室“同龄”的病人。但是因患者开始透析的年龄不同、原发病不同、营养状况也不同，随着透析龄的增加，各种并发症也会不断出现，所以个体差异较大。 透析是否意味着生命进入最后倒计时？ 许多肾脏病患者谈透析色变，认为接受透析治疗后就会彻底变为“废人” ， 生命也进入了最后的倒计时，为此很多已经进入尿毒症期的肾病患者仍坚决拒绝接受透析治疗。 这当然是一种错误的观念！ 目前世界上有肾病患者通过透析维持生命达30年，国内也有不少患者维持10年以上。 透析是一种替代疗法 ，病人通过透析治疗代替本已失去功能的肾脏，维持肾脏最起码的生理功能活动。 肾功能衰竭患者不应该害怕透析，而应该根据专业医生的建议及时接受透析治疗。 常用的透析方式及优缺点 1、血液透析 血液透析可更快改善血浆溶质成分，更快去除体内水分。 高效、方便是其主要优点。 但因为是间断进行，每天去除液体及溶质必须在短时间内完成，所以使内环境波动较大，快速纠正电解质可能导致心律失常，且危重患者往往难以耐受快速脱水，因此并不是所有患者均适合血液透析治疗。 2、腹膜透析 腹膜透析可以全天24小时进行治疗 ， 这种连续性的特点特别适用于一些血流动力学不稳定的患者，如老年人、部分严重糖尿病患者、心功能极差的患者。 对于存在腹部手术后的广泛粘连患者不适宜进行腹膜透析。 当然，具体选择何种透析方式还需要依据患者本人的具体病情、家庭居住环境、居住地的医疗环境、个人医院及医生专业判断来做出决定。 肾功能衰竭患者应该将透析纳入自己的日常生活，像吃喝睡觉一样平常对待。 透析技术日新月异，已不再是仅仅延长患者生命，而是让其更接近于正常状态下继续生存。 因此对于一个处于相对 健康 状态下的尿毒症患者，没有理由认为他不能再工作、再 娱乐 ，至于日常生活就更不成问题，规律治疗后患者的生活质量会更高。 文章 ：山西省中西医结合医院 肾病一科 侯燕琳 配图：网络（侵删） 2019总结：我是一名肾衰竭患者，目前靠洗肾维持生命。风风雨雨的这五年多时间，一路走来实在是不容易。不管经历多少苦难告诉自己不放弃，不管精神和肉体承受多少痛苦告诉自己要坚强，不管将来会是怎样的结局告诉自己坦然面对。撸起来袖子大步往前走，乐观的活着才是王道！加油吧！致自己！#2020我的新年新愿# 医学不问中医西医，有效就行，如果没有透析技术，有些人的坟头草恐怕长了十几年了。中西医取长补短才是正道。 应邀回答问题:人们认为透析了是生命的倒计时，其实人生下来就是生命的倒计时了。只不过是时间长短而已，如果说人的肾脏坏了还能不能活，活多久。两种选择西医是透析，中医治疗是服中药，都是能延续生命，是选择哪种医治为好呢？ 有人在肾功能不全时选择了服中药，但是是药三分毒。加上饮食方法几年，发现中医调理只能维持现状不能治愈肾脏病的慢性肾衰竭。中医中药加饮食调理，尝实过来的患者只是用苦不看言来形容。 如果选择了透析治疗，几年十几年过来人描述，透析象吃饭，透完吃饭香。透析前是病人，透析后最灵验。多数人选择了透析，那是生命的起点！ 要说生命倒计时，人从出生那一天起生命就已经进入倒计时了，谁也无法预料自己啥时候死，血透病人也一样，也不知道啥时候死，医生也判断不了，既然正常人和血透病人都不知道啥时候死，那何不好好活着，不要整天去惦记着自己什么时候死，心态豁达开朗，就是最好的抗病良药，有些人生了点大病，本来可以有希望治好的，或则说可以继续活着，结果自己心头过不去那个坎，天天愁眉苦脸，担惊受怕的，结果病没有把自己病死，吓都把自己吓死了。我也血透快三年了，这三年建，身边有一些认识的正常人，也因为其他 突发疾病去世了，也有意外事件去世了，所以人的生命怎么好预判呢？人命是由天定，天要你死，你就不得不死，天不要你死，即便是你经历车祸，意外，天灾人祸，你都死不了，所以我们能活着的人就要放开心态，好好的活着，不要去想什么时候死，开开心心面对每一天，相信未来会更好。 我的体验是肾病早，中，睌慢牲期找有着丰富临床经验的专科中师看没错！只要找对中医师用对中医。治疗效果强于西医药。遇一病友在中医学院就医，初诊时血肌肝高达780.已是尿毒症。中青年医师纷纷让你透折，患者不甘心，最后找了本院的退休老专家开出中药口服加灌肠治疗方刘二年后，患者血肌酐降至300多，化验指标均优于血透。现在仍坚服中药。 这句话说的是不对的，只要人活着就是倒计时
