[心血管系统]心血管性虚脱是严重过量引起肝中毒的一个表现。 [呼吸系统]它能使对阿司匹林过敏患者的支气管痉挛加重，因此，在同时应用此药和复方阿司匹林时应注意此点。严重中毒时可抑制呼吸中枢。 [神经系统]过量致有肝坏死时可出现精神错乱、激动和注意力障碍，但通常不会有意识丧失。 [消化系统]过量可引起急性肝损害，偶伴有肾小管坏死，伴胰腺炎更罕见。有报道3例每天服5～8g共数周，血清肝细胞酶短暂性升高，组织学证明为中毒性肝炎，长期饮酒者对此药有异乎寻常的敏感性。 [泌尿系统]长期服用大剂量对乙酰氨基酚可致有肾病，包括乳头坏死性肾衰。 [造血系统]可以认为对乙酰氨基酚是潜在的诱发血小板减少的药物。 [皮肤]可发生荨麻疹以及偶见皮炎伴瘙痒，有发生固定性药疹者。 相互作用： (1)在长期饮酒或应用其他肝酶诱导剂，尤其是应用巴比妥类或其他抗痉药的患者，长期或超量服用本品时，更有发生肝脏毒性反应的危险。 (2)抗凝药：大量或长期应用本品时，因可减少凝血因子在肝内的合成，有增强抗凝药的作用，故抗凝药的用量应根据凝血酶原时间进行调整。 (3)长期大量与阿司匹林、其他水杨酸盐制剂或其他非甾体类消炎药合用时(如每年累积用量至 1000g，应用 3年以上时)，可明显增加肾毒性(包括肾乳头坏死、肾及膀胱癌瘤等)的危险。 (4)与抗病毒药齐多夫定(zidovudine)合用时，由于两药可互相降低与葡糖醛酸的结合作用而降低清除率，从而增加毒性，因此应避免同时应用。 二氟尼柳(Diflunisal)使血中对乙酰氨基酚浓度升高。口服避孕药加速它在肾脏的排泄，而降低它的作用。能诱导肝酶的药物使它的中毒更严重。它与香豆素类药合用，使凝血时间更延长。

一、适应病症上的区别：1、乙酰水杨酸：阿司匹林对血小板聚集的抑制作用，因此阿司匹林肠溶片适应症如下：降低急性心肌梗死疑似患者的发病风险预防心肌梗死复发中风的二级预防降低短暂性脑缺血发作（TIA）及其继发脑卒中的风险降低稳定性和不稳定性心绞痛患者的发病风险动脉外科手术或介入手术后，如经皮冠脉腔内成形术（PTCA），冠状动脉旁路术（CABG），颈动脉内膜剥离术，动静脉分流术预防大手术后深静脉血栓和肺栓塞降低心血管危险因素者（冠心病家族史、糖尿病、血脂异常、高血压、肥胖、抽烟史、年龄大于50岁者）心肌梗死发作的风险。其他用途：用途一：解热镇痛药，用于发热、疼痛及类风湿关节炎等。用途二：是应用最早，最广和最普通解热镇痛药抗风湿药。具有解热、镇痛、抗炎、抗风湿和抗血小板聚集等多方面的药理作用，发挥药效迅速，药效肯定，超剂量易于诊断和处理，很少发生过敏反应。常用于感冒发热,头痛、神经痛关节痛、肌肉痛、风湿热、急性内湿性关节炎、类风湿性关节炎及牙痛等。是《国家基本药物目录》列入的品种。乙酰水杨酸也是其他药物的中间体。2、对乙酰氨基酚用于感冒发热、关节痛、神经痛及偏头痛、癌性痛及手术后止痛。本品还可用于对阿司匹林过敏、不耐受或不适于应用阿司匹林的患者（水痘、血友病以及其他出血性疾病等）。二、不良反应上的区别1、乙酰水杨酸（1）胃肠道症状胃肠道症状是阿司匹林最常见的不良反应，较常见的症状有恶心、呕吐、上腹部不适或疼痛等。口服阿司匹林可直接剌激胃黏膜引起上腹不适及恶心呕吐。长期使用易致胃黏膜损伤，引起胃溃疡及胃出血。长期使用应经常监测血像、大便潜血试验及必要的胃镜检査。应用阿司匹林时最好饭后服用或与抗酸药同服，溃疡病患者应慎用或不用。增强胃黏膜屏障功能的药物，如米索前列醇等，对阿司匹林等非甾体抗炎药引起的消化性溃疡有特效。（2）过敏反应特异性体质者服用阿司匹林后可引起皮疹、血管神经性水肿及哮喘等过敏反应，多见于中年人或鼻炎、鼻息肉患者。系阿司匹林抑制前列腺素的生成所致，也与其影响免疫系统有关。哮喘大多严重而持久，一般用平喘药多无效，只有激素效果较好。还可出现典型的阿司匹林三联症（阿司匹林不耐受、哮喘与鼻息肉）。（3）中枢神经系统神经症状一般在服用量大时出现，出现所谓水杨酸反应，症状为头痛、眩晕、耳鸣、视听力减退，用药量过大时，可出现精神错乱、惊厥甚至昏迷等，停药后2～3天症状可完全恢复。大剂量时还可引起中枢性的恶心和呕吐。（4）肝损害阿司匹林引起肝损伤通常发生于大剂量应用时。这种损害不是急性的作用，其特点是发生在治疗后的几个月，通常无症状，有些患者出现腹部的右上方不适和触痛。血清肝细胞酶的水平升高，但明显的黄疸并不常见。这种损害在停用阿司匹林后是可逆的，停药后血清转氨酶多在1个月内恢复正常，全身型类风湿病儿童较其他两型风湿病易出现肝损害。阿司匹林引起肝损害后，临床处理方法是停药，给予氨基酸补液、VitC及肌苷等药物，口服强的松，症状一般在1周后消失。（5）肾损害长期使用阿司匹林可发生间质性肾炎、肾乳头坏死、肾功能减退。长期大量服用该品可致氧化磷酸化解耦联，钾从肾小管细胞外逸，导致缺钾、尿中尿酸排出过高，较大损害是下段尿中可出现蛋白、细胞、管型等。有人认为，部分肾盂癌是滥用阿司匹林等止痛药的继发性并发症。（6）对血液的影响阿司匹林通常不改变白细胞和血小板的数量及血细胞比容、血红蛋白的含量。但长期应用阿司匹林可导致缺铁性贫血。（7）心脏毒性治疗剂量的阿司匹林对心血管没有重要的直接作用。大剂量可直接作用于血管平滑肌，而导致外周血管扩张。中毒剂量可通过直接和中枢性血管运动麻痹作用而抑制循环功能。（8）瑞氏综合征阿司匹林应用于儿童流感或水痘治疗时可能引起瑞氏综合征。瑞氏综合征是一种急性脑病和肝脏脂肪浸润综合征，常常发生于某些急性病毒性传染病以后。病因尚不明确，但普遍认为与下列因素有关：如病毒（流感病毒和水痘病毒）、水杨酸盐、外源性病毒如（黄曲霉素）、内在代谢缺陷等，各因素可相伴存在或各因素间相互影响而造成。临床上病毒性感冒时不主张使用阿司匹林。（9）交叉过敏反应对该品过敏时也可能对另一种水杨酸类药过敏。但是对该品过敏者不一定对非乙酰化的水杨酸类药过敏。2、对乙酰氨基酚少数病例可发生过敏性皮炎（皮疹、皮肤瘙痒等）、粒细胞缺乏、血小板减少、高铁血红蛋白血症、贫血及肝、肾功能损害等。三、用药禁忌上的区别：1、乙酰水杨酸（1）禁用情况有出血症状的溃疡病或其他活动性出血时；血友病或血小板减少症。溃疡病或腐蚀性胃炎；葡萄糖6磷酸脱氢酶缺陷者（该品偶见引起溶血性贫血）；痛风（该品可影响其他排尿酸药的作用，小剂量时可能引起尿酸滞留）；肝功能减退时可加重肝脏毒性反应，加重出血倾向，肝功能不全和肝硬变患者易出现肾脏不良反应；心功能不全或高血压，大量用药时可能引起心力衰竭或肺水肿；肾功能衰竭时可有加重肾脏毒性的危险。（2）慎用情况有哮喘及其他过敏性反应时对于甲亢危象而引起高热的病人慎用（3）限制或禁止服用对阿司匹林、布洛芬、拿百疼等药物过敏者。蚕豆症患者，因为阿司匹林可导致溶血。有肾病、胃溃疡、糖尿病、痛风症等的患者，必须得到医生的许可才能服用。儿童和青少年，不宜使用阿司匹林治疗伤风感冒。血友病病人或其他出血倾向者不宜服用。孕妇。2、对乙酰氨基酚对本品过敏者禁用。肝肾功能不全者慎用。孕妇及哺乳期妇女慎用。

在眼的屈光系统中，角膜的屈光力占全部屈光力的 70%，角膜屈光力的轻度改变即能明显影响近视的度数。PRK及LASIK两种手术正是通过切削中央角膜，使之变薄而降低其屈光力来达到矫正近视目的的。 PRK多应用于治疗中低度近视，但由于破坏了角膜的正常解剖结构，术后可出现角膜上皮下雾状浑浊、青光眼或高眼压、眩光和回退等并发症。LASIK可以保持前部角膜组织的正常解剖结构，能够减轻术后角膜组织愈合反应所引起的上皮下浑浊和屈光回退，预后性较好，术后恢复和稳定性也较好，适合于中高度近视和近视散光的治疗。但LASIK也可能出现并发症，如感染、欠矫或过矫、角膜穿透、医源性角膜散光、继发性圆锥角膜、角膜瓣不规则、眩光等等。这些并发症如果及时发现并处理得当，大部分不会留下后遗症，也不会影响疗效。但是有些并发症确实妨碍视力恢复，比如术前近视术后过矫成高度近视；或术前无散光，术后成为高度散光等等。如果手术致存留的角膜太薄，则无法采用再次手术予以补救。又如，术中角膜穿透或术后继发严重的圆锥角膜，都可能令患者不得不接收角膜移植手术，给患者带来新的麻烦。
