And Ive talked about responsibility a lot. 　　Ive talked about teachers responsibility for inspiring students and pushing you to learn. 　　Ive talked about your parents responsibility for making sure you stay on track, and you get your homework done, and dont spend every waking hour in front of the TV or with the Xbox. 　　Ive talked a lot about your governments responsibility for setting high standards, and supporting teachers and principals, and turning around schools that arent working, where students arent getting the opportunities that they deserve. 　　But at the end of the day, we can have the most dedicated teachers, the most supportive parents, the best schools in the world -- and none of it will make a difference, none of it will matter unless all of you fulfill your responsibilities, unless you show up to those schools, unless you pay attention to those teachers, unless you listen to your parents and grandparents and other adults and put in the hard work it takes to succeed. Thats what I want to focus on today: the responsibility each of you has for your education. 　　I want to start with the responsibility you have to yourself. Every single one of you has something that youre good at. Every single one of you has something to offer. And you have a responsibility to yourself to discover what that is. Thats the opportunity an education can provide. 　　Maybe you could be a great writer -- maybe even good enough to write a book or articles in a newspaper -- but you might not know it until you write that English paper -- that English class paper thats assigned to you. Maybe you could be an innovator or an inventor -- maybe even good enough to come up with the next iPhone or the new medicine or vaccine -- but you might not know it until you do your project for your science class. Maybe you could be a mayor or a senator or a Supreme Court justice -- but you might not know that until you join student government or the debate team. 　　And no matter what you want to do with your life, I guarantee that youll need an education to do it. You want to be a doctor, or a teacher, or a police officer? You want to be a nurse or an architect, a lawyer or a member of our military? Youre going to need a good education for every single one of those careers. You cannot drop out of school and just drop into a good job. Youve got to train for it and work for it and learn for it. 　　And this isnt just important for your own life and your own future. What you make of your education will decide nothing less than the future of this country. The future of America depends on you. What youre learning in school today will determine whether we as a nation can meet our greatest challenges in the future.

(一)治疗精神分裂症的治疗中，抗精神病药物起着重要作用。支持性心理治疗，改善病人的社会生活环境以及为提高病人社会适应能力的康复措施，亦十分重要。一般在急性阶段，以药物治疗为主。慢性阶段，心理社会康复措施对预防复发和提高病人社会适应能力有十分重要的作用。1.抗精神病药物治疗抗精神病药物，又称神经阻滞剂，能有效地控制精神分裂症的精神症状，40多年来广泛应用于临床，明显提高了精神症状的缓解率和精神病患者的出院率。有作者统计近100项(AFLehman，1998)双盲对照，发现抗精神病药物能对50%～80%左右精神分裂症阳性症状有明显疗效，而对照安慰剂仅5%～45%左右。最常用的抗精神病药物，最早在20世纪50年代发现的有以氯丙嗪为代表的吩噻嗪类(Phenothiazine)药物;继之出现以氟哌啶醇为代表的丁酰苯类(Butyrophenone)，以及以氯普噻吨(泰尔登)为代表的硫杂蒽类(Thioxanthene)药物。按临床作用特点，可分为低效价和高效价两类。前者以氯丙嗪为代表，以镇静作用强、抗兴奋和抗幻觉妄想作用明显、锥体外系副作用较轻、但对心血管和肝功能影响较明显为特点，治疗剂量较大。第二类以氟哌啶醇、三氟拉嗪为代表。此类药物的抗幻觉、妄想作用较突出，锥体外系副作用较严重，无镇静、抗兴奋作用，对内脏功能的副作用较轻。此外又发现苯甲酰胺类(Benzamide)的舒必利(sulpiride)亦是有效的抗精神病药物。非典型抗精神病药物氯氮平有明显镇静和抗精神病症状的作用，而锥体外系副作用甚轻。其主要副作用是可出现粒细胞减少甚至缺乏的副作用。一度在国际上停止使用，但以后发现氯氮平对难治性精神分裂症的疗效优于前几类抗精神病药物而又开始在临床应用。但需要长期监测白细胞总数及分类，尤其在治疗早期。此外，氯氮平的发现引起了神经生化和精神药理学界对5-HT拮抗作用在治疗效果以及神经阻滞剂耐受性方面的关注(FleischnackerWW，1999)。为减轻上述抗精神病药物的副作用，出现了第二代抗精神病药物，如既作用于DA受体，又作用于5-HT受体的利培酮(Risperidone)以及与氯氮平化学结构类似，但没有白细胞缺乏副作用的药物奥氮平(Olanzapine)，以及其他如佐替平(Zotepine)和奎硫平(Quetiapine)等。药物的选择，应考虑到临床症状特点以及病人的躯体状况特点。(1)急性期系统药物治疗：首次发病或缓解后复发的病人，抗精神病药物治疗力求系统和充分，以求得到较深的临床缓解。一般疗程为8～10周。常用抗精神病药物的剂量如下：①氯丙嗪：治疗剂量一般为300～400mg/d。60岁以上老年人的治疗剂量，一般为成人的1/2或1/3。②奋乃静：除镇静作用不如氯丙嗪外，其他同氯丙嗪。对心血管系统、肝脏和造血系统的副作用较氯丙嗪轻。适用于老年、躯体情况较差的患者。成人治疗量40～60mg/d。③三氟拉嗪：药物不仅无镇静作用，相反有兴奋、激活作用。有明显抗幻觉妄想作用。对行为退缩、情感淡漠等症状有一定疗效。适用于精神分裂症偏执型和慢性精神分裂症。成人剂量20～30mg/d。④氟哌啶醇：是丁酰苯类药物。本药能较迅速地控制精神运动性兴奋，有抗幻觉妄想作用，对慢性症状亦有一定疗效。锥体外系副作用较明显。成人治疗剂量12～20mg/d。⑤氟哌噻吨(三氟噻吨，复康素)：本药对阴性症状效果较好。剂量10～20mg/d。日量在20mg以上时，易出现锥体外系副作用。对造血系统、肝、肾无毒性作用。起效较快，2周内见效。⑥珠氯噻吨(氯噻吨，高抗素)：本药对妄想、幻觉以及兴奋、冲动等行为障碍效果较好。每片10mg，治疗剂量40～80mg/d。起效较快，1周可出现疗效。⑦氯氮平：因其有明显抗精神病作用，而锥体外系副作用甚轻，而开始应用于临床。其主要缺点是可出现粒细胞减少甚至缺乏的副作用，出现率约1%左右。需要定期检测，治疗开始2个月需每周1次，3个月后需每2周1次。一旦出现粒细胞减少，应立即停药。国内外双盲研究资料显示，氯氮平对急性精神分裂症症状疗效与氯丙嗪等相等同，但对难治性精神分裂症疗效优于氯丙嗪(MeltzerHY，1995)。国外对难治性精神分裂症6周双盲、多中心协作资料(Kane，1988)，发现氯氮平600mg/d的疗效优于氯丙嗪1200mg/d，前者20%有效，后者为4%。常用治疗剂量300～400mg/d。⑧舒必利(Sulpiride)：临床总疗效与氯丙嗪相接近，对控制幻觉、妄想、思维逻辑障碍有效外，对改善病人情绪、与周围人接触亦有治疗作用。治疗平均剂量600～800mg/d。20世纪90年代以来，出现了第二代新型抗精神病药物。这类药物的药理作用不仅限于D2受体，同时作用于5-HT2受体及其他受体。其特点是锥体外系副作用等明显低于第一代。⑨利培酮(维思通)：是5-HT2/D2受体平衡拮抗剂。其主要优点是锥体外系副作用较轻，除对妄想等阳性症状有效外，亦能改善阴性症状。国内多中心研究发现，利培酮治疗精神分裂症对阳性症状及阴性症状均有效，患者对该药的耐受性及依从性也较好(顾牛范，1998)。成人治疗剂量为3～4mg/d，个别可达6mg/d。⑩奥氮平(奥兰扎平)：作用于D4、D3、D2受体及5-HT2、α2受体。较氯氮平的优点是无粒细胞缺乏的严重副作用，无锥体外系副作用。国内临床研究发现，奥氮平对阳性、阴性症状以及一般精神病态均有良好疗效，锥体外系不良反应少(舒良，1999)。成人治疗剂量为5～20mg/d。76长效针剂：适用于有明显精神症状而拒绝服药或有藏药企图的病人，以及处于巩固疗效、预防复发维持治疗阶段的病人。治疗剂量：属于吩噻嗪类药物的有癸氟奋乃静(氟奋乃静癸酸酯)12.5～25～50mg，每2～3周肌内注射一次;棕榈酸哌泊塞嗪(哌泊噻嗪棕榈酸酯)，50～100mg，每3～4周肌注1次;属于丁酰苯类的有癸氟哌啶醇(安度利可长效针剂)50～100mg肌注，每2～4周1次;五氟利多30～40mg口服，每周1次。少数病人可用至每周120mg。此外，硫杂蒽类的长效针剂：癸氟哌噻吨(三氟噻吨癸酸酯，氟哌噻吨癸酸酯)每2周肌注20～40mg，对改善慢性症状有效，且疗效较快。癸酸珠氯噻醇(癸酸氯哌噻吨)，每2周肌注200mg，对改善阳性和阴性症状均有效。维持治疗：癸氟奋乃静(氟癸酯)12.5～25mg，每4～6周肌注一次;棕榈酸哌泊塞嗪(哌泊噻嗪棕榈酸酯)50～100mg，每4～8周肌注1次;或癸氟哌啶醇50～100mg，每月肌注1次;或口服五氟利多10～20mg，每周1次。药物的剂量因人而异。一般从小剂量开始，逐渐加量，速度因个体对药物的耐受情况和对药物的敏感性而异。一般于10天至2周内加至治疗剂量，一般在治疗量4～6周内可控制急性精神分裂症症状。对有明显自伤、伤人、兴奋躁动病人，宜迅速控制兴奋，防止病人发生意外。人工冬眠常温治疗(氯丙嗪和异丙嗪各50mg肌注，2次/d)或快速氟哌啶醇肌注治疗(3～4次/d，每次5～10mg)，与口服常规给药相比，有疗效短、显效快、安全的优点。约50%病人在两周内明显改善精神症状。氯丙嗪可引起低血压副作用，老年血压高者慎用。(2)继续治疗和维持治疗：①继续治疗：在急性期精神症状业已得到控制后，宜继续用抗精神病药物治疗剂量持续1个月左右，以期使病情获得进一步缓解。然后逐渐减量进行维持治疗。②维持治疗：旨在减少复发或症状波动而再住院。现有的双盲研究资料表明药物维持治疗对预防本病的复发十分重要。Kane总结了21篇1970～1986年发表的双盲和安慰剂对照的有关研究，证明采用抗精神病药物的维持治疗对减少复发或再住院十分有价值。最近有一大宗临床观察，在急性症状控制后的第1年，如服用抗精神病药物，复发率20%～25%，服安慰剂者为55%。另有作者(Hegarty等)报道维持治疗3年观察，发现抗精神病药物维持治疗组在预防复发上较安慰剂组高2～3倍。长效制剂的疗效和口服制剂无差异。间断治疗一出现症状就加药的效果，不如连续药物治疗。维持治疗的时间一般在症状缓解后不少于2年。如病人系复发，维持治疗的时间要求更长一些。这一阶段的抗精神病药物逐渐减量，以减至最小剂量而能维持良好的恢复状态为标准。一般在3～6个月后逐渐减至治疗量的1/2，如病情稳定，可继续减量，减至治疗量的1/4或1/5。如病人为第2次发作，药物维持的时间更长一些。即使用较低剂量维持，定期复查，随时调整剂量，可避免复发。(3)剂量、疗效和副作用：抗精神病药物治疗中注意药物副作用。对儿童、老年人和躯体疾病及脑损害者治疗剂量要偏低。当前倾向抗精神病药物剂量不宜偏大。国内抗精神病药物临床疗效、剂量与血药浓度测定研究(舒良等)发现高剂量氟哌啶醇0.4mg/(kg99d)和低剂量0.15mg/(kg99d)临床疗效相同，但前者副作用大于后者。最近国外PET的研究资料(表9)表明抗精神病药物的低剂量(如氟哌啶醇5mg)基底神经节D2受体80%被阻滞。这种低剂量已能在大部分病人身上产生抗精神病的作用。剂量加大可能增加镇静作用，但这同时副作用相应增加，特别是锥体外系副作用及其有关处理较费事的迟发性多动症。故国外有作者建议为进一步控制兴奋，可合并短期苯二氮卓类等药物控制兴奋。从上表可以看出，200mg氯丙嗪，300mg硫利达嗪(甲硫达嗪)，4mg氟哌啶醇，800mg舒必利可使基底神经节DA受体的75%～80%受体阻滞。(4)合并治疗：原则上应尽可能使用一种抗精神病药物。有必要时，可将低效价和高效价神经阻滞剂合并使用，但宜以一种为主。抑郁症状在精神分裂症病人可见，可能来自疾病本身，或是心理反应。有报道奥氮平(Olanzapine)能减轻抑郁症症状。比较奥氮平(Olanzapine)(5～20mg/d)和氟哌啶醇(5～20mg/d)对精神分裂症症状和抑郁现象的疗效，发现前者对抑郁症状的疗效明显高于后者，除部分系由于阳性、阴性症状的改善，锥体外系反应消失外，部分是由于对情绪改善的直接作用(TollefsonGD，1998)。当抑郁症状严重时，可合并抗抑郁药物治疗。中药银杏叶提取物Ginkgo-biloba制剂(商品名舒血宁，每片40mg，含Ginkgo-biloba9.6mg)，系抗氧化剂，对脑功能有改善记忆、延缓衰老的作用。北京医科大学经临床多中心对照研究验证，发现合并原有抗精神病药物，治疗6～8周后能改善慢性精神分裂症症状。剂量银杏叶提取物80～120mg，3次/d，疗程8～12周(罗和春，1997)。其作用机制有待研究。长期使用大剂量神经阻滞剂，易出现迟发性运动障碍，目前尚缺乏有效治疗方法，应尽量避免其发生。故应：①尽可能用最低有效剂量，保持最佳效应;②避免用超大剂量;③尽可能少用抗胆碱能药物;④早期识别主要副作用，及时调整药物种类或剂量;⑤北京医科大学精神卫生研究所的双盲研究临床验证口服异丙嗪(非那根)25～50mg，3次/d，或肌注异丙嗪50mg，2次/d，数周可明显减轻迟发性运动障碍症状(杨旭东，1999;杨甫德，1998)。抗锥体外系副作用的药物，如苯海索，宜在副作用出现后才合并使用。2.当前抗精神病药的新进展(1)概述：自从20世纪50年代氯丙嗪问世以来，各国先后研制了多种抗精神病药，为数成百上千，形成了好几种类别和系列。但是，这些药物对精神分裂症的疗效，实际上都没有超过氯丙嗪的水平，这种状况直到出现了氯氮平以后才有所改观。我国早在1970年就已经开始在临床上广泛应用氯氮平。从30年的应用经验看来，可以认为此药疗效既好，锥体外系不良反应又少，而且它导致粒细胞缺乏的可能，实际上可以预防。对于这一成功经验，虽然我国医生自己没有清楚地意识到它的重要性，但却着实震惊了各国精神科医生和制药业。为此，Sandoz药厂特地资助Kane进行研究，把268例曾经应用多种抗精神病药治疗而未见疗效的难治性精神分裂症患者，随机分成两组，一组用氯丙嗪，另一组用氯氮平治疗。结果发现，氯丙嗪组只有4%好转，而氯氮平组却有37%见效，两者相差悬殊。从此，美国FDA才给氯氮平的临床应用“开禁”，但还是心有余悸，规定在使用时必须留有余地，要求每周验血以防粒细胞缺乏的发生。正如我国临床医生的经验一样，各国医生对氯氮平的疗效赞叹不已，但是对于那么多的限制和次数繁多的验血，也实在感到不便。与此同时，氯氮平的出现又澄清了另一个问题：锥体外系反应与抗精神病药的疗效并没有直接关系，也就是说，锥体外系反应并不是治疗所必需;氯氮平几乎没有锥体外系不良反应，而疗效却很好。另外，氯丙嗪等药，尤其是舒必利，由于较多地持久地阻断了多巴胺受体，从而大大增加催乳素水平，影响了正常的内分泌功能，导致性功能和月经的异常。氯氮平则不同，它与多巴胺受体结合不多，也不持久，在这方面的不良反应就明显减少。(2)新一代抗精神病药：各国医药界纷纷研究氯氮平之所以疗效较好、而锥体外系不良反应却较少的原因，一般认为有以下几种可能，或许可以作为研制新抗精神病药的依据。①氯氮平既能阻断多巴胺D2受体，又能阻断5-HT2受体：从这种理论看来，似乎作为理想的抗精神病药，其阻断5-HT2受体的作用应该越大越好，与5HT2/D2结合的比例应越高越好(即抑制常数的比例应该越小越好)。从表10和表11可以看到各种抗精神病药与受体结合的资料。表11所示都是与受体结合的抑制常数，数值越小，说明它与受体的亲和力越大。氯氮平与D2的结合，只是氟哌啶醇的1/125，利培酮为氟哌啶醇的1/3，奥氮平则是1/11。由此可以看出利培酮的锥体外系不良反应仍较多的原因所在。在表10中，还可以看出氯氮平和奥氮平的抗胆碱能不良反应及嗜睡作用较大，就因为它们与M及H1的结合，远远高于氟哌啶醇等药，也高于利培酮。②氯氮平较少结合多巴胺D2受体，较多结合多巴胺D4受体：如果这是氯氮平疗效较好的关键所在，那么作为理想的新抗精神病药，其与D4受体的结合应该越多越好，D4/D2结合的比例应越大越好(即抑制常数Ki的比例越小越好)。从表12可以看到各种抗精神病药的D4/D2抑制常数的比例，除了已被禁用的普拉平外，以氯氮平和奥氮平的数值最小，似乎可以说明问题。但是有人研制合成了专门与D4结合的药物，却对精神分裂症根本没有治疗效用。因此看来D4并不一定是关键所在。③氯氮平的特点可能在于它能结合其他受体(如NMDA)：若确是如此，寻找理想抗精神病药就得另辟蹊径。最近有人在研究谷氨酸与精神分裂症的关系，就是基于这一可能。④氯氮平虽然结合D2，或许其亚型和效应并不相同：舒必利似乎就是如此，但其疗效却不尽如人意。舒必利的几个同类药物都因为不良反应等问题，最终未能进入临床。看来这条途径并不一定能够如意。在以上这些说法中，最为流行的是第1种。不少制药集团就从这个理论出发，研制了多种新的抗精神病药，例如奥氮平、利培酮、奎硫平、舍吲哚和齐拉西酮等(注：舍吲哚因致QT延长，已遭FDA禁用)。为了有别于氯丙嗪之类的经典抗精神病药，往往把氯氮平和这些新药一起统称为“非典型抗精神病药”。归纳起来，其特点有3个：A.很少或没有锥体外系不良反应，很少或没有迟发性运动障碍。B.对精神分裂症的阴性、阳性和认知症状的疗效都比较好。C.不增加或很少增加催乳素。自20世纪90年代以来，非典型抗精神病药已自成一族，由于它们的不良反应较少，疗效较好，因此比较受患者欢迎。其用量已占总用药量的相当大部分，在有些地方已超过一半以上。(3)理想抗精神病药：作为一种理想的抗精神病药，应该有以下条件。①安全：首先应该是安全，对于心、肝、肾等脏器没有毒性。②不良反应少：各种不良反应都应该比较少，最好没有或很少有锥体外系不良反应;特别应该没有不可逆的不良反应，如迟发性运动障碍。③疗效好：不论对于阳性、阴性，还是认知症状，都应该有效。④可以长期维持服药以预防复发：药物适合长期应用，并能通过维持服药来有效地预防精神病的复发。⑤服法简单、方便：最好能够一步到位，一下子就达到治疗剂量，不必逐步调整。如果能有长效作用，就更理想。⑥价廉物美：因为精神分裂症需要终身服药，药价是一个很实在的问题，不能简单地把疗效较好、生活质量较高、能够及早恢复工作等作为借口，套用所谓宏观的卫生经济学计算方法来掩盖药价昂贵的事实。(4)抗精神病药物的疗效：抗精神病药如能奏效，原有的精神分裂症症状理应完全消失，而且患者应当恢复自知力，这才称为“痊愈”。经典的抗精神病药虽然相当有效，但毕竟仍有30%～40%的病例在用药后未见显效(症状评分减少一半以上)，而且即使有效，其中60%仍有较多残留症状。著名的国际精神分裂症研究(IPSS)在9个国家和地区进行了细致的调查，发现精神分裂症最多见的症状是“缺乏自知力”，97%病例都有这种表现。至于在治疗之后能否恢复自知力，似乎最有关系的因素可能是病程长短和症状缓解是否充分，与抗精神病药的种类也有一定关系。除此之外，精神分裂症的症状主要表现在以下4个方面：阳性症状、阴性症状、情感症状和认知缺陷(图1)。①对阳性症状的疗效：阳性症状主要包括妄想、幻觉、怪异行为以及阳性思维形式障碍。前三者也就是所谓的“精神病性症状”，后者是指除了思维中断之外的思维形式障碍。已如前述，即使对于阳性症状，经典抗精神病药的效果也不够理想。目前看来，还是氯氮平的疗效最好，明显优于氯丙嗪、奋乃静或氟哌啶醇。但是是否所有新的抗精神病药都是如此，目前还不敢定论。至于它们的疗效是否优于氯氮平，更有待临床验证。②对阴性症状的疗效：阴性症状包括：A.情感淡漠，也就是在情感的体验和表达上都有所缺陷。B.思维贫乏，以致言语的质和量都减少。C.对环境和事物的兴趣减退。D.社交缺陷，往往趋于孤独。E.注意力难以集中。虽然眼下时行用量表来对阴性症状笼统地进行评定的方法，但实际上问题并非如此简单。应该说，阴性症状还有原发与继发之分。原发性阴性症状是精神分裂症的病因本身所造成，也可称为“缺陷综合征”(defectivesyndrome)。继发性阴性症状则可能出于以下几种原因：其一，阳性症状(如幻觉妄想)有可能引起类似阴性症状的继发性表现。例如患者幻听丰富，就会对外界环境无动于衷，给人以情感淡漠的印象。随着幻听的减轻，这种“阴性症状”也就会有所好转。因此可以说，随便哪一种抗精神病药恐怕都会有这种功效。其二，精神分裂症的情感症状(例如抑郁)也有可能在量表上被误评为情感淡漠。其三，抗精神病药的锥体外系不良反应(例如面无表情、运动不能等)更有可能被评为“阴性症状”。因此，在应用锥体外系不良反应较重的抗精神病药时，往往会出现“越是治疗，‘阴性症状"越重”的现象。最后，还有一部分阴性症状实际上是环境单调所造成。有些患者在离开医院进入社区生活后，阴性症状竟然明显好转，就是这个道理。总的说来，精神分裂症初发病例，只有10%显示原发性阴性症状。如果没有得到较好的治疗，原发性阴性症状还会明显增多。据统计，30%～60%的慢性病例都有不同程度的原发性阴性症状。现有的抗精神病药对原发性阴性症状能否奏效，还是一个值得研究探讨的问题。如今，有些报道认为氯氮平或一些新的抗精神病药都有治疗阴性症状的功效，实际上很可能只是它们能够消除阳性症状，从而使原来阳性症状所引起的类似阴性症状那样的继发性表现有所好转;而且它们较少引起锥体外系不良反应，所以“阴性症状”评分较少，而不一定是真正地对原发性阴性症状有效，见图2所示。由此看来，对于这种宣传介绍，必须予以慎重剖析和反复验证，不宜贸然轻信。③对情感症状的疗效：大约30%精神分裂症病例伴有抑郁或消极意图(甚至自杀行为);有的与阳性症状相伴发生，有的出现于阳性症状缓解之后，后者往往称为“精神病后抑郁”。据统计，至少10%精神分裂症患者有自杀意图或行为，当然也可能是由于幻听等阳性症状，其原因不一定是抑郁。对于精神分裂症所伴抑郁，有必要进行仔细分析。如果它是精神分裂症本身症状的一个部分，患者对于这种抑郁症状必然没有自知力，也不要求予以治疗纠正;对于这种抑郁，抗精神病药应该可以奏效。相反，如果患者对于自己的抑郁情绪具有自知，而且迫切要求治疗，那么，这种抑郁很可能是药物或心理反应所引起;可以采用的措施应该是：更换药物品种，并用抗抑郁药，或者干脆予以电休克治疗。值得一提的是：有些医生遇到没有自知力的抑郁患者，感到难以诊断。如果说是抑郁症，患者没有自知力，觉得不够典型;如果说是精神分裂症，又缺乏标准典型的症状。于是往往就给予所谓“不标准的抗抑郁药”舒必利来进行治疗。结果情况有所好转，就此流传出“舒必利可以治疗抑郁症”的说法。实际上，舒必利所治疗的是“伴有抑郁的精神分裂症”。就这样，有的病例在服用舒必利半年或1年后，按抑郁症的治疗常规予以停药，结果病情便急剧恶化，出现典型的幻觉妄想;这一点恳请临床医生注意和重视。如今尚有传说，某种新抗精神病药可以治疗抑郁。实际上不少病例就在应用这些药物以后、精神分裂症症状得到明显好转的同时，出现了精神病后抑郁。如果这些药物真能治疗抑郁，那就不应该出现这种现象。因此，对于这种说法必须慎重对待，必须分清此“抑郁”是精神分裂症症状表现的一个成分，还是真正的抑郁症。对于后者，恐怕还是应用抗抑郁药治疗为妥。④对认知缺陷的疗效：认知功能是指从环境、过去经历和其他方面获得感知信息，并进行组织和加工利用的过程，一般是指记忆、思维、语言和解决问题等方面的能力。以前总认为精神分裂症并不影响认知功能，因此不会殃及智能。其实不然，据张明园等调查，正常老年人的认知功能测验CASI总分为92.0，老年性痴呆为31.8，而老年精神分裂症患者为60.5，说明精神分裂症确实会影响认知，影响智能。不少研究发现，即使是年轻精神分裂症患者的智能也较差。有人怀疑这是长期住院的单调生活所致，但是因其他疾病长期住院的患者却没有这种现象。由此看来，认知缺陷是精神分裂症本身症状的一部分，尤其是慢性病例更显严重。从现有资料看来，经典抗精神病药并不能改善认知缺陷，而却有可能因为药物阻断了多种受体，反而使之加重。此外，另有文献报道抗胆碱能药(如苯海索)也会加重认知缺陷。至于氯氮平等新抗精神病药是否真正能够使之改善，还有待设计良好、不带偏见的研究予以验证和澄清。不少学者指出，对于那些由药厂资助的研究结论，应该慎之又慎，切勿轻信。根据前述那种“既阻滞D2又涉及5-HT2才能提高疗效”的理论，应该说只有利培酮或奥氮平那样的药物，才具有可与氯氮平相媲美的疗效。但是有些药物，如五氟利多和氟哌啶醇，对于5-HT2并没有显著的阻断作用，却也有很好的效果，这又该如何解释?看来，问题并非如此简单，所谓的D2-5-HT平衡学说不见得就是真理。此外，究竟该不该应用“靶症状”这个概念?“某种药物对某种‘靶症状"特别有效”，这种说法有无根据?从理论上看来，如果某种症状有它自己的特殊病理机制，而且又有能够针对这种病理机制的药物，那么才可以说这种症状是该药的“靶症状”。精神分裂症有它自己的病理机制，或许就在D2，或许就在5-HT2，或许同时在于两者，或许在于其他方面;但无论如何是一个统一的病理机制。正如大叶性肺炎是一种疾病，是一个整体;青霉素或头孢类抗生素是针对病因的治疗，止咳药只是辅助药物。医师绝对不会、也不应该单用止咳药来治疗大叶性肺炎。也没有哪一个内科医师相信青霉素是针对高热这个“靶症状”的药物，或者相信头孢类抗生素是针对咳嗽这个“靶症状”的药物。在内科医师眼里，青霉素或头孢类抗生素都是针对病因――肺炎链球菌的治疗药物。同样地，治疗精神分裂症，也不应该单用“专门治疗幻听”的药物(如果有的话)，正如“头痛医头，脚痛医脚”那样。如果阳性或阴性症状确实各自有其特殊的病理机制，那么或许能找到可以分别治疗阳性或阴性症状的特殊药物，把它们中间的一个作为“靶症状”。遗憾的是，目前还未能阐明阳性或阴性症状各有什么样的特殊病理机制。如果阴性症状确实是额叶功能减退，是额叶的多巴胺通路不畅，那么能够选择性地增加额叶多巴胺的药物，就将是能够针对阴性症状这个“靶症状”的特效药物。很可惜，现有的抗精神病药都具有同样或类似的药理机制。所以从理论上说，不可能存在各种不同的“靶症状”。有的药物如氯丙嗪或氯氮平，嗜睡副反应较大，所以在应用于兴奋躁动病例时，似乎效果较为明显;其实，没有嗜睡副反应的氟哌啶醇，对兴奋躁动病例同样有效。所以，不应该把兴奋躁动说成是氯氮平的“靶症状”。按照同理，也没有根据说某药所针对的靶症状是幻觉或妄想……也就因为如此，近年来已不提倡“靶症状”的说法;这些新一代抗精神病药，也不再强调各自能治疗哪种靶症状了。基于同样理由，目前在国际上很流行的“治疗谱”的说法，也是不正确的概念。精神分裂症的阳性、阴性症状，与细菌的阳性、阴性类别，是完全不同的两种概念，不应该混为一谈，也不应该生搬硬套地借鉴过来随便应用。现在很流行的说法是：“某某药是广谱药，某某药对阳性、阴性症状都有效”，实际上与“靶症状”是同样的谬误。其实，抗精神病药与抗生素不一样。抗生素有专治革兰阳性菌的，有专治革兰阴性菌的。如能兼治两者的，称为广谱抗生素。抗精神病药并非如此，从来没有哪一种抗精神病药是专治阴性症状的，也没有哪一个抗精神病药是专治阳性症状的;一般都是阳性症状先奏效，然后阴性症状才有所好转。实际上，有经验的精神科临床医师都知道：应用抗精神病药治疗精神分裂症，第1个见效的症状是兴奋躁动，其次是幻觉，然后是妄想、思维形式障碍，再后是情感淡漠，最后才是自知力的恢复。所以说，这实际上是一个见效的“层次”问题。临床经验告诉我们，在治疗之初，各种药的效果都不相上
