（一）体位:本手术须经腹部及会阴部两处进行，所以应取膀胱截石位，同时麻醉后将手术台摇成头低足高位，使手术台与水平面大约成15°角左右，这样有利于腹腔内的小肠移向上腹部，能很好的显露下腹腔及盆腔，有利于操作。截石位时的躯体与大腿之间、大小腿之间的角度均应摆在舒适的功能位置上，以防止损伤髋关节及膝关节周围的韧带及损伤腓总神经。（二）消毒：消毒应包括腹部及会阴部两部分，腹部上至乳头水平，会阴部直达大腿的上2/3。以碘酊、酒精消毒、或以络和碘、洗必泰均可，阴囊最好用新洁尔灭消毒，女性患者应进行阴道消毒。然后铺足够厚度的无菌单。（三）切口：以选择腹部正中切口最方便，同时有利于两侧淋巴结的清除。术者应站在患者的左侧，切口自耻骨联合向上，左侧或右侧绕脐直达其上10cm左右。本人的体会是以左侧绕脐较为方便，有利于左侧结肠的游离。因造瘘口的位置低于脐，不会增加污染机会。（四）切开腹壁：在上述选定的切口处，切开皮肤、皮下脂肪、腹白线，向下切开锥状肌，直达耻骨联合。然后采用透光照射的方法切开腹膜前脂肪及腹膜，向下推开膀胱，以免损伤。同时腹膜切开尽量在中线，这样有利于关腹。如果想一侧推开膀胱，偏向另一方切开腹膜，则给关腹时缝合腹膜增加一些困难。（五）腹腔及盆腔检查：开腹后应注意有无腹水，依次检查肝脏有无转移以及转移的程度，仔细检查全部腹膜有无播种结节，检查大网膜上有无转移结节，再依次检查上腹部的胰腺、脾、胃等以及双肾，再沿腹主动脉及肠系膜下动脉走行探查上方及侧方淋巴结有无肿大，以及植物神经有否受浸润，髂血管有无受浸润等，最后探查原发病灶，主要确定病灶的位置是否已侵犯周围的组织以及与其他器官和骶前筋膜的关系，然后再根据术前检查综合判断采用根治性术式的切除范围。（六）腹腔无瘤技术：探查结束后，开始手术前，首先以大块纱垫将所有小肠推向上腹腔、使之与术区隔离。再以纱布带在肿瘤的上方结扎直肠上部，以防止肠腔内的脱落癌细胞沿肠壁向上移动，这样以纱布带结扎后提起直肠也有利于操作。找到肠系膜下静脉向其中注入5-Fu 250mg，向纱布带以下的直肠腔内注射5-Fu 250mg。（七）游离乙状结肠及直肠上段系膜及清除淋巴结：助手将乙状结肠提向右侧，此时常常可见乙状结肠系膜与侧盆壁（腹壁）之间生理性粘连，逐一分离之。然后在乙状结肠系膜左侧的最低部剪开一小孔，以血管钳提起后，如无严重浸润，腹膜即与后面的组织分开，用长剪刀在腹膜下作钝性分离出一隧道后扩大腹膜开口。向下直达腹膜返折处后，再向前转折，剪开直肠膀胱（或直肠子宫）凹表面的腹膜。接着游离松解腹膜后的结缔组织，并找到左侧输尿管，以纱布带提起，同时游离出左精索动静脉，以免损伤。将乙状结肠翻向左侧，在腹主动脉的前面剪开乙状结肠系膜的右侧叶，同样以剪刀先作钝性分离后，剪开或是以剪刀切开右侧叶，向上达肠系膜下动脉根部，向下直达右侧腹膜返折处，并与在左侧腹膜的剪开处会合。在下腔静脉的右侧缘处找出右输尿管，以纱布带提起，以免损伤。当游离开了乙状结肠左、右侧系膜后，在肠系膜下动脉与腹主动脉的夹角中，术者以左手的食指及中指从右侧向左侧深入，将肠系膜下动脉牵直，助手将乙状结肠展平。此时术者以左手仔细触摸，判定肠系膜下动脉的走行及其分支的走行，并确定其自腹主动脉发出处，剪开腹主动脉前结缔组织，直达腹主动脉的固有膜，显露出肠系膜下动脉根部，将其周围的结缔组织全部清除，自根部向下，以左手引导扶持，清除沿该动脉走行的结缔组织并清除显露出乙状结肠诸分支及直肠上动脉，在终末支乙状结肠动脉分出的下方结扎切断直肠上动脉；也可以在肠系膜下动脉根部2 cm处结扎切断。自肠系膜下动脉根部高度，在其左侧约2cm处可找到肠系膜下静脉清除其周围的结缔组织，在与肠系膜下动脉相同的高度结扎切断之。分离切除左右髂总动脉前的脂肪淋巴组织。（八）游离直肠：按照全直肠系膜切除术(total mesorectalexcision，TME)原则游离直肠．在直肠固有筋膜与盆壁筋膜间隙内进行，勿损伤下腹下神经、盆神经丛及骶前静脉丛。先分离其后部及侧部，下达尾骨尖及两侧提肛肌平面，再分离直肠前方至前列腺尖端平面。1）前方的游离：淋巴结清除结束后，以血管钳将盆底腹膜最低点剪开处的前叶夹起，助手将前面的膀胱或子宫阴道用深部拉钩向前上，此时可见到稍呈坚韧的膜状组织，即为Denonvillies筋膜，在此筋膜的前方进行游离，即可见到精囊腺与前列腺（精囊距肛缘的高度约为5cm，前列腺上缘距肛缘高度约为4cm），向下沿此间隙进行游离，尽量达低位（因腹部游离越低，会阴部操作越容易），如无明显浸润，可达前列腺的下极，只要层次认准，很少有出血。2）直肠后方的游离：清除淋巴结之后，在腹主动脉分叉部的下方，仔细解剖出骶前筋膜，在骶前筋膜的壁层与脏层之间进行游离。以电刀或长剪刀自上而下在此间隙中游离，此时可以见到位于骶前筋膜深面的骶中动脉及骶前静脉丛，注意一定不要损伤骶前筋膜的壁层及脏层，否则易造成癌细胞的扩散及难以收拾的大量出血。如癌肿无明显的浸润，则骶前间隙仅为疏松的结缔组织，很容易分离，当达到第3骶椎水平时，可见到比较坚韧、致密的Waldeyer筋膜（在直肠后壁固有筋膜的第3骶骨水平，与骶前筋膜之间连接着一膜状的筋膜，称之为Waldeyer筋膜，也称骶骨直肠筋膜）同样以长剪刀靠近骶前筋膜剪断之，否则易损伤直肠壁。继续向下游离直达肛提肌处。后壁的游离一般不至于造成出血。3）侧壁的游离：沿后壁游离的边缘向左右侧延伸，沿髂内动脉向下游离。当达腹膜返折下约2cm处，髂内动脉与直肠侧壁之间可见到稍坚韧的结缔组织，即直肠侧韧带，锐性剪断之（靠近血管处），然后清除膀胱直肠间隙中的结缔组织。同时于根部结扎切断直肠中动脉。至此，直肠的游离结束。接着分两组同时进行腹壁造设人工肛门及会阴部的操作。（九）会阴部的操作：会阴部操作之前，一定缝闭肛门。切口：会阴部切口不宜过小，其界限为：前为会阴中点，后为尾骨尖，两侧为坐骨节结。作一椭圆形切口，先以手术刀切开皮肤，环行一周，然后术者与助手分别牵开皮肤的内缘与外缘，用电刀切开皮下脂肪组织，直至显露出臀大肌前缘及肌纤维，两侧达坐骨结节，及会阴浅横肌的后缘为界线，继续向深层以电刀清除坐骨直肠凹中的脂肪组织，直达肛提肌的下方。在坐骨直肠凹脂肪中的最深处有直肠下动脉自髂内动脉延续而来，其走行方向为由外向内，于根部切断结扎之。切断肛提肌：先在直肠与尾骨之间，切断肛管尾骨韧带，然后逐渐向两侧沿骨盆的侧壁，在肛提肌的止点处，依次切断肛提肌中的髂骨尾骨肌、耻骨尾骨肌及耻骨直肠肌。至此，会阴部操作中的后面及两侧的创面均可与腹腔中游离的创面相通。此时可将乙状结肠的断端及要切除的肛管自直肠后壁相通了的孔道翻出至肛门外。以左手用力前拉提出的肛管，并以拇指及食指将前壁剩余的相连部分组织夹住，从前壁切开的创面中仔细解剖出前列腺包膜（女性为阴道后壁）与Denonvilliers筋膜之间的界限，自外向内仔细游离，可用电刀进行。逐步向内进行，直达会阴体部，先于会阴浅横肌后方切断肛门外括约肌浅部的交叉纤维，向深进行达会阴深横肌及耻骨直肠肌，沿会阴深横肌的后缘切断耻骨直肠肌。此时可根据触摸下的导尿管位置的位置，判定尿道的正常位置，进行操作，以免损伤。切断耻骨直肠肌后，可见到直肠尿道肌，其后方连接直肠纵肌，前方连接尿道膜部。判定正确位置后切断。再向上切断会阴三角韧带，可与腹部创面相通，然后将标本移除。直肠前壁的肿瘤，常常使Denonvilliers筋膜与前列腺的包膜或女性的阴道后壁粘连甚至浸润，此时行前壁游离时，常常造成前列腺包膜撕裂出血，或阴道后部外膜的出血，因该部的静脉丛丰富，极易出血，所以，①游离时一定要找好层次，尽量从腹部进行游离，这样层次可能会更清楚，尽量减少会阴的游离；②一旦发生该部出血，应尽量缝合止血，单纯结扎或电灼往往不确切，不易奏效；③如果肿瘤浸润，可行前列腺或阴道后壁的部分切除，然后进行前列腺的缝合或阴道修补。（十）腹壁造设人工肛门：当在预定线处切断乙状结肠后，会阴部操作开始的同时，腹部手术组即可进行人工肛门的造设。目前国内外均采用经腹膜外一次性开放造瘘法。造口部位的选择：常选择的造口的部位有二种，一种为脐与左侧髂前上棘连线的中外1/3，另一种为左腹直肌外缘与髂前上棘与其连线交点的外上角处。首先在腹腔内在预定造瘘口的高度，提起乙状结肠，剪开处的外侧腹膜作潜行游离，上下约10cm左右，直绕过前腹壁，大约达腹直肌中线水平。术者以左手深入潜行隧道中，作引导，然后在预定造瘘口处，消毒，助手以止血钳提起，预定造口部的中点，半径约2cm，作圆形切口，切开皮肤，在以电刀与该处腹壁相垂直切除一直径为4cm的圆柱形脂肪，深达腹直肌前鞘或腹外斜肌腱膜，长度与皮肤切除范围相当。在十字型分开腹直肌或切开腹外斜肌，显露出腹直肌后鞘，或腹横筋膜，将其十字切开，切开的长度要充分，但也不宜过大。不充分，会使肠道内容物通过受阻，过大也易造成切口旁疝。此时应不要损伤已游离的侧腹膜。用4把血管钳子将腹直肌后鞘提起。经已游离的侧腹膜外将拟行造瘘的乙状结肠拉出腹壁之外，拉出时要注意乙状结肠不可扭曲，应使乙状结肠系膜对准耻骨联合。拉出肠管的断端一般以高出皮肤2~3cm为宜。然后将乙状结肠的相应高度先在深层与腹直肌后鞘作一圈缝合固定，注意勿损伤乙状结肠的血管，影响造瘘肠管的血运。再将肠管的断端与皮肤的真皮层作一周环形，外翻缝合，针距不宜过大，宜缝合12~16针为宜。另外，因作外翻时系膜缘外因系膜脂肪较多，不易外翻，因此缝合时，应尽量偏低，而其他部位稍高，这样外翻后，造瘘口一周肠管高度一致。 缝合固定侧腹膜与拉出的乙状结肠，以封闭肠管外侧的孔隙以免造成内疝，先将剪开的乙状结肠外侧的侧腹膜的上部斜行盖于乙状结肠上，作间断缝合固定，再将其下方的切缘与乙状结肠系膜缝合固定。注意盖于乙状结肠上面的腹膜一定要松弛，不可勒住了乙状结肠，至此，人工肛门造设结束。（十一）关闭腹腔及会阴部创口：上述操作结束后，反复冲洗腹腔，冲洗液自骶前创面中流出。最后再以含有5-Fu的盐水冲洗，冲洗后检查有无出血点，一一结扎止血后，缝合剪开的后腹膜。闭合盆底腹膜时应注意：①针距不可过大，以不大于0.8cm为宜，否则极易造成内疝；②宜提起两侧的腹膜，在其边缘缝合，如缝合过多时，一是易误缝输尿管，二是缩小了腹腔而增大了骶前腔的容积，延迟骶前腔的愈合；③勿将盆底腹膜缝合成很尖的圆锥形，而尽量缝合成一比较圆滑的创面，否则小肠移至盆腔后易粘连成角造成梗阻。后腹膜缝合完后，清查腹腔并将全部小肠按顺序置回原来的位置，再将大网膜覆盖其上，清点器械、敷料无误后逐层关闭腹腔。同时会阴组人员于骶前腔里放置胶管引流二枚，在一侧或二侧坐骨节结的内侧另开口引出。最后封闭创口（可一层缝合，如果允许也可二层缝合）。固定引流管，会阴创面如果过大，缝合时有张力，估计一期愈合有困难时，也可以不置引流，不予以缝合，而以大块凡士林纱布敷贴创面后，以大纱布填塞，手术结束。术后2~3天，拔出大纱布，然后以1/5000的PP溶液反复冲洗，坐浴，待其自然愈合。

　　Kind of Blue，如果你不知道这张唱片，会想到什么？有一点忧伤？也没错，它确实很适合在一个当你感到有一点忧伤的夜晚聆听，事实上，这个标题解释了这张唱片的一切，为什么它被认为包含了爵士乐所需的一切元素。最关键的那个元素就是，Blues!蓝调。作为爵士乐的前身，蓝调音奠定了爵士乐的基础，让它区别于古典乐。所有的爵士乐从某种意思上都是kind of blues。Miles是在一天晚上听到教堂里的福音音乐为灵感创作了Kind of Blue，而唱片中的全部五首曲子中也有两首是典型的12小节蓝调。拿Kind of Blue作为爵士乐的入门专辑来听也是再适合不过了。　　对于这张伟大的作品，史上卖得最好的爵士唱片之一，赞美之辞也不用我多说了，毕竟在美妙的音乐面前，一切语言都显得空白乏味。作为一个业余爱好者，在此列出一些我所了解到的信息以及自己的一些感受，若有新的想法以后再更新。下面就开始，从这张唱片给我的几个最强烈的感受说起吧。　　Personel乐队成员　　这张唱片是以六重奏的配置录制的，光是看看封面上列出的乐手，就让人倒吸一口凉气，这简直就是全明星的“银河战舰”啊。小号Miles Davis，次中音萨克斯John Coltrane，中音萨克斯Cannonball Adderley，钢琴Bill Evans，贝斯Paul Chambers，鼓James Cobb。每个人都是独当一面，日后也都成了band leader开创自己的领域，绝对是爵士史上最强大的阵容之一。这当然要归功于Miles的领导才能以，Miles总是能把最好的乐手聚集到一块，提供一个施展其天赋的平台；另一方面时机也是非常合适，想想如果两年之后再想聚齐这些人马难度就非常之大了。　　其实这个时期Bill Evans已经离开Miles的六重奏去准备自己的三重奏了，Wynton Kelly取代了他的位置。Miles为了这张专辑的Modal实验（有关mode调式后面会提到）又把Bill请回来。也许是考虑到现任钢琴手Wynton Kelly的情绪以及发挥Wynton的蓝调才能，Miles在最有蓝调味的曲子Freddie Freeloader里启用了Wynton。据说Wynton来到录音室看到Bill的时候很诧异，他以为他是来录制整张专辑的，结果被告知：两人都有份，先一边等着去。　　Spontaneity自发性　　录制分别在两天进行。1959年的3月2日录制了A面的3首，4月22日完成了B面的两首。几乎所有的曲目都是录制一遍完成的，除了Flamenco Sketches录制了两遍。这在录音史上是很罕见的。不仅如此，Miles只是在录制前一天晚上准备了一些曲目和弦结构以及旋律，第二天发给乐手再做一些简介。在没有排练的情况下直接进行录制，乐手们并不知道具体要录制什么样的音乐，只是根据Miles提供的有限资料进行演奏和即兴。这似乎是Miles一贯的作风，少排练，以最大程度发挥乐手的想象空间，并以此种方式展示爵士乐的自发性，和听众一起分享创作的瞬间。在Flamenco Sketches里，乐手只拿到五个用来即兴的音阶而已，连一条旋律都没有！Miles相信第一次尝试往往是最好，First thought was the best thought. 而恰恰是在one take, no reheasal（无排练一遍录制）的情况下，一张杰作诞生了。为何称之为杰作，以下继续说明。　　Simplicity简化　　1953年钢琴家George Russell提出了Modal improvisation调式即兴，即以某个音阶作为即兴的基础，取代以和弦为主的即兴方式。而Miles Davis作为从Bebop（比波普）成长起来的乐手，却对Bebop不满，认为其过于复杂频繁的和弦变化约束乐手的即兴空间，有碍爵士的发展。而Modal的方式恰恰简化了即兴的基础，只需要几个音阶即可。所以Miles成了最早实践调式即兴的音乐家之一。Bill Evans在George Russell那学习了调式即兴，最早在1958年和Miles在milestone这首曲子上做了尝试。而这次的Kind of Blue则是完整地建立在调式即兴上的。So What只有两个和弦，Flamenco Sketches只有五个音阶。这种简化，加上上面提到的无排练录音方式，最大程度地给予了乐手空间。而乐手个个都是大师级的人物，在这种初次尝试里自然地发挥了无限的想象力和潜能。　　抛开理论，光从音乐的演奏本身来讲，也能体会到这种simplicity。每首乐曲的旋律线都是很简单的，Miles和Bill的演奏风格也是具有空间感的，并不紧凑，有时一个长乐句里只有几个音符，less is more。其实对于刚听爵士或者接受不了爵士的人来说，最大的困难便是听不懂到底是在演奏什么，也听不出结构，似乎只是随音地飙音符而以，谈何享受呢？而Kind of Blue却是很easy listening的，在后面具体的乐曲分析里，我会解释每首曲目的结构，进一步分析这种简化的效果。　　Combination结合　　前面说到Miles的一个特长便是笼络天才乐手，而在Kind of Blue里我们也可以听到黑人爵士和白人爵士的结合。Bill作为这个六重奏里唯一的白人，显得含蓄而内敛，Miles甚至形容他像一个nonperson（隐形人？），安静地坐在角落里。鼓手James Cobb回忆到有一次Miles和他们在车里聊天，Bill在一旁静静地听，突然来了点意见想发言，Miles开玩笑地说：We don"t need any white opinions.而Bill对这张唱片的贡献不仅仅是一个sideman了，其营造的氛围和Bill的风格有很大的关系。　　乐手间的风格差异和互动是很有意思，看一眼乐谱或者靠耳朵听，那个音符最少的一定是Miles的即兴，最多的一定是John Coltrane。Coltrane在这一时期的即兴风格有一个专门的词，叫做sheet of music，就是在短时间内加入大量上行或者下行的琶音，个人感觉有一股喷射出音符串的力量。Cannonball的也很有自己的风格，尤其在蓝调方面。　　Miles的另一个过人之处便是乐于创新，every time he made a change，he was tight，并且改变爵士乐的发展方向。Kind of Blue的发行影响了当时几乎所有的爵士乐手甚至是流行乐，以及一代接一代的音乐家。　　Mood（心情音乐？）　　好的音乐我想不仅仅是好听，而是动听，能够带动你的情绪，沉浸在音乐的氛围里，上瘾，这张唱片就是这种类型。神秘，诗意，沉静，忧伤，性感。Herbie Hancock说：if you want a kind of music to make love to, that"s it.如果夜晚驾车时放上这张唱片，一定很销魂。　　唱片中营造的氛围也和录音室的条件有关。录音室在纽约的30th Street Studio，之前用作教堂，空间很大，而且都是上了年代的木头，拥有很好的回音效果，很多传奇的唱片录制于此。一个好的录音室是有精神的，就像伦敦的Abbey Road Studio，恰巧Pink Floyd的音乐也是以营造氛围出名的。（好吧，实际上这两个录音室没什么关系，我只是借机宣传一下喜欢的乐队，但是如果你听Dark Side of Moon和Wish You Were Here，你会发现和Kind of Blue是有相似之处的，就是一种特容易让人沉浸的感觉）　　下面具体讲讲五首乐曲。　　先说说爵士乐的基本结构，一般说来，开头先由全体乐手演奏主旋律（有时也会在主旋律前也会有introductoin）；之后由各个soloist（每个乐手都可以成为soloist，当然小号和萨克斯是最常见的）进行以主旋律的和弦或者音阶即兴演奏，结构不变；等soloists结束即兴，再回到主旋律，演奏一遍之后结束。　　So What　　最出名的曲目，之后cover的版本非常多，为什么？简单。全曲只有两个和弦Dm7和Ebm7，对应的是D dorian音阶和不过全部上移半个全音的Eb dorian。主旋律结构是16小节Dm7，接着8小节Ebm7，再回到8小节的Dm7，总共32个小节。这样给即兴的空间就很大了。也许这曲名有点这种意味：就是这么简单，怎么的了（so what?）　　开头有8小节的introdution，很神秘，只有贝斯和钢琴，接下去便进入32小节的主旋律。主旋律采用的是贝斯(Chambers)和三个管乐(Davis, Coltrane, Adderley)的call and response呼应的方式，先有贝斯旋律，接着三个管乐有两个音符的回应。主旋律结束之后先由Miles进行两遍（64小节）的即兴，接着Coltrane的两遍即兴，Cannonball的两遍即兴（你应该很明显听出演奏风格的区别了，另注意Coltrane的次中音和Cannonball中音的音色差别），最后是Bill的一遍即兴。回到主题，结束。　　Freddie Freeloader　　曲名出自Miles的一个乐迷叫Freddie，他经常不付钱去观看Miles的演出（他是怎么做到的？我也想当freeloader!）　　上面说到此曲是典型的12小节Blues，很欢快，主旋律很简单。钢琴上由Wynton Kelly取代了Bill，在唯一录制的曲目上，Wynton也是演奏得相当给力，主题之后便是他的4遍即兴，接着Miles6遍即兴，Coltrane5遍，Cannonball5遍，Chambers2遍贝斯即兴，回主旋律。　　Blue in Green　　虽然专辑上是把所有曲目的创作归于Miles名下的，但这首Blue in Green被认为是Bill Evans所作，而且充满了Bill的浪漫气息。在Bill诗意的4小节intro之后，是10小节的主题，Miles的演奏同样非常简单，一个音符都不多用！此曲Cannonball是没有参与录制的，实际上是五重奏，Bill, Coltrane和Miles分别进行了即兴。另外此曲也是整张专辑里最短的，相比其他曲目都有十分钟左右的长度，它只有5分36秒，和较短的主题和少以为soloist有关。但感受到的却同样是漫长的宁静，似乎听着就要睡着了！（现在3点了我还在听着这么催眠的曲子奋战）　　All Blues　　看名字就知道了，另一首12小节蓝调作品。注意开头的贝斯（有一个非常funky的旋律）以及Bill的伴奏方式，4小节之后两个萨克斯加入伴奏，Miles最后进来负责主旋律。即兴部分，Miles，Cannonball和Coltrane分别做了4遍即兴，接着Bill弹了两遍，最后回主题。　　Flamenco Sketches　　此曲的特别在于，干脆连旋律也不要了，给出五个音阶，直接进行即兴，乐手可以根据自己的感觉在一个音阶上演奏任何长度再跳入下一音阶直到演奏完所有音阶，最大程度地展示了调式即兴的自由的特点。在贝斯和钢琴的4小节intro之后，便进入即兴，Miles, Coltrane, Cannonball, Bill最后再回到Miles。　　作为唯一录制了两遍的曲目，此曲的另一版本在97年重新发行的CD里作为bonus track出现了。两遍将近20分钟的长度，听者真的是要完全陷进去了，催眠良药。
